Share your insights and expertise with your peers! Share your great work with the CJA community and spark new developments to improve health in your community.

Conference breakout sessions will provide attendees with resources and opportunities to learn from innovators and leaders to develop strategies and identify resources and tools for their work. In addition to the breakout sessions, breakout presenters are encouraged to register, at a discounted rate, for the entire conference.

Proposal Topics

Breakout sessions will fall under one of three tracks listed below, and should also focus on building health, well‐being and/or achieving health equity. We are especially interested in presenting programs that demonstrate solutions with replicable results. We also want to hear and learn from presenters who can share obstacles that evolved on your path towards improving health and achieving health equity in your community, and approaches for resolving obstacles or barriers.
Breakout Session Tracks
Track 1: Community level interventions and programs – proposals should focus on successful implementation of models, coalitions, initiatives or programs for achieving health equity, population health improvement; demonstrating interdisciplinary or multi‐ sectoral community health strategies and health improvement. Highlight lessons learned and ways to adapt or replicate the intervention or program.
Track 2: Financing health improvement and measuring value – proposals should focus on pay for performance, community benefit, impact investing; , approaches for measuring and evaluating program or initiative impact, or other evidenced/promising practices to measure improvement and finance specific programs or initiatives.
Track 3: Promoting health equity –proposals should demonstrate a particular focus on serving vulnerable or underserved populations; strategies that address addressing racism by promoting inclusion; improving conditions and cross sector collaboration (health, economic, education, housing, faith, social, and community). Examples could include community health worker programs, health homes, community centered health, public health collaboratives, and/or pathways hub programs.

Submission Guidelines

Each 1.5‐hour session will consist of two presentations, with up to two speakers from each presenting organization(s). CJA reviewers will assign each presentation to a session with a related topic or field of work.

PLEASE NOTE: All speakers are required to register in order to confirm participation and encouraged to remain for the entire conference to continue networking and sharing ideas.

Timeline

Submission deadline: September 16, 2016.
Selection Committee decisions will be announced October 14, 2016.
Conference registrations open September 21, 2016
Upon approval, presenters will be able to receive a $25 conference registration discount. There will be a limited number of scholarships available and CJA reserves the right to award scholarships.
