CALL FOR PRESENTATION PROPOSALS

October 22-23, 2009
Hyatt Regency Austin

 

CJA is inviting proposals for the break-out learning intensives

All proposals are due August 14, 2009

We want to ensure a quality series of experiences from local communities who are moving toward and achieving 100% Access/Zero Disparities in their community.   To enhance the learning experience of participants, we are very interested in receiving proposals from health access coalitions and professionals who have been successful at implementing a program in one of the following focus topics.

Focus Topics

·         Community Benefits

·         Measures/Metrics That Worked

·         Local Implications of Health Policy, Health Reform and the Stimulus

·         General Best Practices

General best practices can come from one of the 8 critical activities* that are necessary to achieve 100% health care access or from other successful programs you may have initiated to achieve zero disparities. 

*Outreach & enroll into eligible programs; Provide culturally & linguistically competent medical homes; Provide affordable prescription drugs;  Organize access to specialty & hospital care; Manage chronic diseases; Connect to prevention & wellness services;  Coordinate care continuum; and Develop strategies to provide access/coverage to low-wage workers.

SUMMARY: 

We are inviting community coalitions and professionals to submit proposals to be an integral “teaching” participant in one of our breakouts.    Communities selected to share their experiences will be included in the conference program.

INFORMATION REQUIRED IN A SESSION PROPOSAL

  • Which of the focus topics (you may submit more than one) you are implementing.
  • The title of the proposed presentation within the intensive session dedicated to this topic.
  • Presentation synopsis (500 word limit) which includes:
    1. Very brief description of the community and the coalition and the need for this activity.
    2. A clear description of the outcome that you wanted to achieve.
    3. A brief description of the “program” you planned to implement to achieve this focus topic.
    4. The challenges you encountered in implementing the program.
    5. Modifications you (may) have made to the program.
    6. How you are measuring and reporting on the outcomes you are achieving.
    7. Lessons you have learned that will help other communities more quickly and effectively implement a similar program.
    8. What you would like to learn from others to enhance your effectiveness.
  • Whether your program is in the planning stage, early operational, or fully operational stage.
  • Name, credentials (degrees), job title, mailing address, email address, phone number and membership status for each presenter.
