Overview of Presentation:
 
Institute for Healthcare Improvement (IHI) Triple Aim organizations pursue better health for populations, improve experience of care, and lower per capita costs. There has been an evolution within organizations focused on the IHI Triple Aim from sub-populations to a geographic region. In this webinar, Dr. Whittington will explore IHI's current thinking on the distinctive issues and opportunities presented by regionally-defined Triple Aim improvement, including high-level learning from several IHI Triple Aim regional sites. In addition, he will discuss how this work supports national initiatives such as those focused on care coordination and accountable care organizations.
 
Learning Objectives:   From this one and a half hour webinar, participants will be able to
 
- Understand the framework used for regions striving to accomplish the IHI Triple Aim
- Gain lessons learned from IHI Triple Aim regional sites
- Describe the linkages between this work and national initiatives

Dear Members and Friends of Communities Joined in Action,

Happy Holidays! Communities Joined in Action (CJA) has had an exciting and productive ten years! This year alone, we have accomplished two major milestones: hosting a successful conference, and hiring our first CEO, which we achieved through a CORE Operations Challenge grant from the California Endowment. We are now ready to embark upon our next strategic phase designed to strengthen the reach of CJA, and we need your support.

As you know, healthcare for the most vulnerable in our society is least accessible. A myriad of solutions are emerging, however, that expand access to healthcare, improve patient outcomes, and lead to better utilization of resources. Much of this innovation is being driven by community health collaboratives (CHCs), which develop coordinated systems of services by connecting medically-underserved people to needed health care services. This coordination improves the quality of care and results in significant savings that can be reinvested into the system to further expand access to healthcare services.

To quote Dr. Marilyn Gaston, former CJA Board member, and Chief of the U.S. Bureau of Primary Care, “We have come a long way, but now we need a revolution.” To this end, we are extending our role in improving access and eliminating disparities, and have launched the Campaign to End Disparities, developed a robust set of learning and capacity-building trainings that will prepare our members for growth and sustainability, and have begun forging critical national partnerships. We have also laid additional groundwork by forming national committees on Metrics, Marketing, Programs, and Public Policy, and preparing for the launch of our new interactive clearinghouse of best practices.

With your donation, you help ensure that those whom we serve continue to receive the innovative programming that provides access to quality, coordinated healthcare. Your support extends to a network of over 1,000 communities who are closing the gap on inequities in health and well-being.

Invitation to Submit a Breakout Proposal

 

The 2011 CJA national conference theme is:   

Improving Health ~ Eliminating Disparities in an Age of Healthcare Reform

Proposals are being requested from local community health collaboratives and organizations that have demonstrated success reorganizing local delivery systems and/or restructuring financing of services while significantly improving health outcomes.

·         Please go to   http://www.cjaonline.net/events/annualConf/2011/index.htm for complete RFP and detailed instructions for submitting proposals online.

  • Proposal submissions due by Thursday, May 26st
  • Selection Committee decisions will be announced on June 22th

 

Each breakout session will address one or more of the following critical activities that relate to improving health and eliminating disparities:

1.      Outreach & enrollment into publicly subsidized programs

2.      Culturally & linguistically competent medical homes

3.      Prevention & wellness initiatives

4.      Affordable prescription drugs

5.      Chronic disease management

6.      Specialty & hospital care

7.      Access and coverage models for low-wage workers

8.      Coordinated care across the continuum of health and social services
