Clare Tanner, PhD, directs the Center for Data Management and Translational Research at the Michigan Public Health Institute (MPHI). MPHI is a non-profit organization based near Lansing, Michigan. Dr. Tanner’s current work focuses on issues related to high quality and accessible primary care; linkages between healthcare, public health and social services; and the role of shared multi-sector data for advancing community collaboration to improve health and well-being.
Dr. Tanner’s group was a co-recipient of a Health Care Innovation award from the Centers for Medicare and Medicaid Services, which implemented the Pathways Community Hub model in communities across Mid-Michigan, targeting adults with multiple chronic conditions. In addition, Dr. Tanner and her team are the evaluators of Michigan’s State Innovation Model program. Dr. Tanner also co-directs the National Program Office of Data Across Sectors for Health, (along with Illinois Public Health Institute), supported by the Robert Wood Johnson Foundation. Dr. Tanner received her PhD and MS degrees in Sociology from the University of Wisconsin-Madison.
