Carly Salamone has served as the Assistant Director of the Northwest Ohio Pathways HUB at the Hospital Council of Northwest Ohio since 2010. The Certified Toledo- based HUB provides care coordination services for pregnant women and women of childbearing age, adults with chronic diseases, breast health patients, and victims of human trafficking. Over the last 5 years, Carly and her team have secured over $12 million in grant funding to address social determinants of health and improve health outcomes among people residing in vulnerable communities. Carly received her Bachelor’s degree in Public Health in 2011 from the University of Toledo and her Master’s in Public Health (MPH) from the Northwest Ohio Consortium for Public Health in 2012.
